Athletic Training

What is an athletic trainer?

Athletic trainers are health care providers that are trained in prevention, assessment, treatment, and rehabilitation of athletic injuries. We are onsite for all DHS athletic events and look forward to working with all DHS athletes and their families. If you sustain an injury when we are not on campus, please come in the next day of your sport and we are happy to see you then.

Specific injury and concussion forms can be found if you scroll down this page.

We would like to thank you all for allowing us to work with your student athletes. If you have any questions, please don't hesitate to contact us, we're here to help in any way we can!

Team Physician

Dr. Carey Eugene Ellis began working with DHS Athletic Trainers in the spring of 2024 and brings a depth of sports medicine knowledge, also serving as a team physician for the Chicago Bears and other professional sports teams in Chicago. Dr. Ellis will provide on-site medical services on Mondays at the Deerfield HS Athletic Training Facility as well as Football games in the fall.

Dr. Ellis sees patients in Lincolnshire and Glenbrook. To book an appointment with Dr. Ellis, please reach out to DHS Athletic Trainers or contact the NorthShore SportsCare Concierge Service at 224-251-2467 and specify that you are a Deerfield HS athlete needing to be seen.
